What else do I need to submit with my application?
You do not need to submit anything else with your application – just make sure that it is completed and signed. In order to process your application, the information on the form itself is all that we need. If your child is accepted (which depends on availability in the grade to which your child is applying at the time you applied), you will then need to provide and complete additional paperwork. See # 10 below for information on the required documentation.
