What are the deadlines for completion of enrollment paperwork?

All returning students must provide proof of residency and a revised lunch form by June 1, 2010 to remain enrolled for the 2010-2011 school year.

For new students accepted before June 15th, we must have proof of DC residency, the national school lunch form, the Chavez Special Needs Questionnaire, and a Home Language Survey by July 1st, 2009.

Any students accepted after June 15th will only have 3 business days from the date of acceptance to complete the above paperwork (demand becomes high in the summer).

For new students, we cannot schedule your child if we do not have a report card showing that they passed their previous grade and/or have enough credits to be in 10th grade (at the HS level). We need to have final report cards (MS) or transcripts (HS) by July 15th, 2009 or else your child’s space will be given to another student if there is a wait list in your child’s grade; or we will schedule your child to repeat the grade that they were in last year if there is not a wait list in your child’s grade. If your child is making up credit in summer school and will not have a final grade after this deadline, please see #13 below.

Both returning and new students must have their immunization and medical records by the first day of school. If the medical records/immunization are not available by the first day, a family, at minimum, must provide documented proof of a scheduled appointment.
